Today I received a letter from Arnold Schwartsnigger saying he is auditing my work and I need to fill out a questionnaire form and mail it back by the end of the month.
The questions are just basic things like "does your employer provide you with the equipment to do your job" ...etc
My question is do I have to fill this thing out and mail it back?? Will there be consequences if I don't?? -

Are you paid by W-2 or are you issued a 1099 at the end of the year? Sounds like they are trying to figure out if you are an actual "employee" or "contract labor". You would rather be an employee b/c you would only be subject to half of your social security and medicare taxes.

W2...i just dont feel like filling the damn thing out and mailing it back seems like a lot of work

do you work for a venue that is owned by a state organization? Since the deaths at the cow palace two weekends ago, the governor ordered a mandatory review of all state-owned venues. This is probably why you got this.

no its just a privately owned restaurant
