Playing 1/3 to pass the time before the FT heads up play started; about 90 minutes into the session we're in the middle of a hand, board is Q<span>?</span>7<span>?</span>8<span>?</span>4<span>?.</span>
I'm not in the hand but I'm watching the dealer and he's looking at one of the burn cards - moving it around, rubbing the back of it, examining it. Then he throws his hand up to call the floor over and shouts "I need a new seven of diamonds over here. New seven of diamonds please." ...
Thankfully the pot was small and the table was casual. -
Never seen that happen, and I play a ton live (400ish hours a year). Everytime a card is bad, the dealer takes out and replaces both decks, as is procedure at Casino Arizona. I don't have a problem with replacing only one card, as it is much quicker, but yeah, you should never alert what card it is in the middle of a hand unless you are using two decks and are using the non foul deck at the time.

I've got two similar stories from the Gutshot in London:
1) Playing tourney and the dealer notices one of the cards is marked. He holds it up for the Tourney Director to see (this is mid hand) in a way that the players at that table couldn't see what the card was. The Tourney Director then yells "You need a new Ace of clubs then yeah!"
2) Playing an all night Ł1/Ł2 cash session and the dealer is obv tired. Deals the cards and the pre flop action gets pretty wild and the pot is well over Ł100. Flop comes out, more betting, pot is about Ł300, three players left in the hand. Instead of dealing the turn the dealer shuffles the flop and remaining cards and starts dealing to everyone again! Needless to say it kicks of big time and the owner of the club is called (this is probably 5am) to give a ruling. I don't remember the exact cards but one guy has top set and another has an up and down straight flush draw!

The reason I don't play at Rio, WSOP, or any Harrah's property a different story was during WSOP. It was a 2/5 NL ring game. The dealer had asked for the floor twice becuase he was simply exhausted and needed relief and need to go home. So here is the story.
There is a ton of preflop action (already like a $500 pot with 1 all in). Dealer deals 1 card flop. OOOOPs (it happens) figures it out drops 2 more cards... 2 more all ins (about a $900 pot a couple side pots) and he drops the deck and almost fouls it before turn and river.
Somehow we figured it out. I got up and left.
