Honestly I think the substance abuse is probably your main issue. I've been married to an alcoholic before. At a certain point no matter how much you care about someone, a substance abuser becomes downright unnattractive. Your wife will be boning someone else and some other guy WILL be raising your Kids soon if you don't get a handle on that. I think once you conquer that issue, everything else will naturally fall into place. She does sound awfully spoiled though. Housekeeper too?
